Job summary

37.5 hours per week. The Community Nursing Team provide nursing care to patients in their own homes in the beautiful setting of the South Hams in Devon. The Community Nursing Service sits within a health and social care team so the work involves working with an integrated approach to care, making every contact count. The team work closely with GP partners, local Hospice's and Voluntary Partnerships. Its an exciting and vibrant opportunity to join the team. This is a great opportunity to gain skills working in the Community Nursing Service.

Main duties of the job

The main duties of the role are to provide nursing care for housebound patients in their own homes. To promote self care and preventative approaches to ill health with focus on wellbeing the emphasis being of preventing hospital admissions and living well.

The post holder will be delivering nursing care to include, wound care, medication administration, bowel care, catheter care, Complex Wound Management, end of life care, managing frailty as part of the MDT approach in preventative care. Using an recognised Nursing Model to assess, plan, implement, evaluate nursing needs of the patient. The use of IT as many records are now completed electronically.
