Specialist Community Public Health Nurse- Health visitor - Wokingham (Band 6)
We are an amazing team and part of Berkshire Health Visiting service covering across the Wokingham area.
If you regard yourself as an enthusiastic and dynamic qualified health visitor then come and join our forward thinking, vibrant team of health visitors in Wokingham. Whether you are an experienced health visitor or newly qualified, we will ensure you receive individualised and effective support from both your health visiting line manager and colleagues within your team.
You will work within the Wokingham area to deliver core and targeted services to children aged 0-5 and their families. You will require excellent interpersonal skills, the ability to work in partnership with other teams and agencies, along with great communication skills and a passion for working with children and families.
A commitment to team working, flexibility and working with partners is essential, as you will collaborate with Children's Centre's in the implementation of the Healthy Child Programme underpinned by the Solihull Approach model of working.

If successful, your base will be Wokingham Community Hospital (Wokingham) however, your role will require you to visit homes across Berkshire (mileage will be compensated).

Main duties of the job

- Develop and implement outcome focused care plans and evaluate care given, in partnership with clients.
- Provide support to families using the Solihull approach model in behaviour management, parenting skills and delivering programmes of care to vulnerable families.
- Develop and facilitate Health Visiting drop-in clinics, health promotion activities and groups.
