The ACERS community heart failure service is tailored to the needs of the City and Hackney population by providing evidence based care for patients in clinics and in their homes, and working closely with the hospital team if they are admitted. The GPs are enthusiastically committed to the model and to providing excellent care networks for patients in partnership with Homerton Hospital cardiologists, health care professionals and other agencies.
We are looking for an enthusiastic, pro-active, self-motivated nurse to join the team as one of the key links in the chain of gold standard care. Essential to this new role is experience of caring for heart failure patients, along with excellent communication skills with which you will develop personal management plans and educate empower patients and carers for increased confidence in self-care. Experience of working in the community is desirable. This post will let your personal and clinical skills blossom as you set new ways of working in place to really make a difference.
Main duties of the job
You will work with the team to support heart failure patients within the community to optimise their heart failure treatment, help them manage their condition, and prevent unplanned admissions. The role will also involve responsibility for and leading on specific areas to ensure that patients have a seamless pathway through different aspects of the service to assist in the implementation of National Guidelines and Quality Standards for chronic heart failure care.
