To facilitate and promote the safe, timely and efficient transfer between acute and community services, advising hospital and community nursing staff and where appropriate, medical colleagues on aspects relating to safe and effective transfers of care provision
To support the development of systems and processes at a ward level that will educate nursing and medical staff alike as to the importance of effective discharge planning being part of patient care. To ensure adherence to agreed discharge standards and collate information to inform continuous improvement of systems, wards, specific patient groups.
To champion a partnership approach with primary and secondary care, social services and voluntary organisations, with an aim of achieving resilient robust discharge activity for all patients no matter where they live.
To manage own caseload of patients requiring complex continuing healthcare
(CHC) assessments through their patient journey, acting as patient advocate and to support family to identifying a suitable discharge destination

Main duties of the job
- To support the wards in the timely completion of appropriate referrals based on patient needs (ongoing social/ health care), planning and management of effective and timely patient discharge.
- To process and distribute discharge support referrals to the relevant discharge support destination.
- To provide a link between ward teams and external partners to promote seamless patient transfers/ discharges.
