Are you a person centred leader with a keen interest in community care? Would you like to be part of the leadership team working in partnership with colleagues and key stakeholders to continue the development of our care community teams ? This is an opportunity to lead one of our 8 care community teams.

We are looking for an enthusiastic and motivated individual who is committed to working with our local populations and wants to develop or consolidate their management skills within a supportive team.

This position is in our Ellesmere Port West Care Community Team for a period of 12 months to cover for a maternity leave.

**We offer a relocation package of up to �8,000**

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership.

To be eligible for the relocation payment, you must live more than 40 miles from the base/location of the post you are applying for and you would need to relocate to within 15 miles of the new base.

Main duties of the job

You will be responsible for all operational management across the team, provide leadership to the team and play an integral role in developing and delivering person centred care.

To lead, operationally manage and develop an efficient and effective care community team delivering multidisciplinary health and social care for adults, older people and their carers. Ensuring that the Trust values, policies, procedures and good practice are adhered to and which ensures that adults and older people receive seamless, evidenced based services.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment.You are encouraged to enrol for the DBS Update Service. A small annual fee of �13 per year applies.

New entrants to the NHS will commence on the minimum of the scale stated above.

Person Specification

Qualifications

Essential

  • Degree (or equivalent experiences) and post graduate diploma (or equivalent)
  • Evidence of Continuous Professional Development via personal development plans/file


Desirable

  • Project Management Qualification
  • Master's degree/working towards a master's degree or equivalent management level experience


Knowledge and Expertise

Essential

  • Proven ability of working at managerial/senior level in an NHS or social care organisation
  • Proven ability of working in a care service, encompassing all aspects of performance management
  • Proven ability of demand and capacity management
  • Experience of developing business cases
  • Demonstratable experience in delivering change through teams, encouraging them in the development and delivery of plans
  • Track record/experience of project management/change management processes
  • Experience of building effective working relationships with staff, immediate management and MDT teams
