Working as part of a Substance Misuse multi-disciplinary team in a community drug service for service users with complex substance misuse needs, the post holder will, using a range of generic and or specific tools be responsible for providing individual service user assessment and identifying, delivering and evaluating care interventions. This will include providing direct care to service users.

The post holder will have a designated caseload of service users, delegated by the Clinical Pathway Manager or designated supervisor. They may also take on a care co-ordination role for a specific group of service users.

Care will be delivered within the framework of NICE guidelines evidence based best practice and models of recovery and abstinence. The post holder will also be responsible for carrying out a range of profession specific or general tasks as part of an identified treatment process or as identified in the service users individual care or recovery plan.

Main duties of the job

Reporting to the Service Manager/Clinical Lead or designated supervisor the post holder will undertake a range of necessary tasks identified within the individual care plan. This will involve delivering care and interventions appropriate to the post holders level of experience and competences without direct supervision.

To contribute to multi-disciplinary comprehensive, culturally appropriate assessments to establish eligibility for health and social care services.

To undertake continuous risk assessment and to contribute to effective risk management

Recognizing and valuing service users as individuals the post holder will ensure that the care provided respects equality and diversity that is supportive of service users and their carers. This will include encouraging service users to accept an optimum level of responsibility for their individual programme of care and with their consent, where appropriate seek the cooperation of friends, relatives and/or carers.

Job responsibilities

As a front line worker the main responsibilities of tis job is working with service users with substance misuse needs. Through assessment , case management , risk assessment and management. Liaison with other professionals and joint care and crisis planning. As part of an MDT you will be expected to review and present cases and facilitate treatment options and offers to service users to support their recovery . Being an effective team worker to contribute to the safe and effective service delivery and service user satisfaction and positive outcomes. This will also include having a think family approach and identifying and supporting safeguarding of children and vulnerable adults.

Qualifications

Essential

  • Current Registration Mental health /registered General nurse


Desirable

  • Mentorship or recognised teaching and assessing certificate


Knowledge and Experience

Essential

  • Previous experience of working in mental health or a substance misuse setting
  • Demonstrable understanding and experience of utilising a variety of assessment tools currently used in practice in relation to determining risk and assessing / planning care needs
  • Demonstrate a clear understanding of current Mental Health legislation and its application / use in practice or legislation relevant to substance misuse services.
