There is an exciting and rare opportunity for an experienced Registered Nurse to join the Discharge Liaison team within the Llandough Centre for Specialist Spinal and Neuro Rehabilitation.
The 26 bedded Spinal Rehabilitation unit cares for and rehabilitates individuals following complex spinal injuries.
The post holder will be a knowledgeable resource within the Specialist Spinal Rehabilitation unit in the field of Discharge Planning, Continuing Healthcare, Delayed Transfers of Care and National and Local policies. You will support integrated professional working to achieve the best provision of care for service users.
Main duties of the job
As a Discharge Liaison Clinical Nurse Specialist, you will manage and support complex spinal discharge planning through a multidisciplinary process which includes assessment, goal setting, planning, implementation, co-ordination and evaluation to prepare individuals and their families with appropriate support and resources as they move from hospital to their homes or to hospitals closer to their homes.
The post holder will be required to have a flexible approach to the needs of the service and team. An exceptional understanding of hospital discharge procedures and experience in working in a multi-agency setting is essential.
