We are looking for a band 6 RMN who is highly motivated, innovative, enthusiastic and passionate about service user person centred care to work within Redbridge Memory Service.

Main duties of the job

Key Responsibilities:
  • Providing assessments and supporting diagnosis for people with Memory problems
  • Comprehensive assessments in the Service Users home
  • Working effectively with the MDT and ROAMHT
  • Facilitating Mental Health Nurse led clinics in the department. To encourage and develop teamwork within the team.
  • Working effectively with our partners e.g., Local authority, Alzheimer's Society and Age UK
  • Excellent knowledge of the Mental health act and Mental Capacity Act
  • To ensure a safe working environment
  • To work with the Team in maintain accreditation from MSNAP
  • Proactively collect feedback from Service Users and their Carers.
  • Cover other teams/services, where necessary, which may require working on other sites
  • Provide managerial and clinical supervision to junior staff
  • To be a practice assessor/ supervisor
  • Education and training for the MDT or junior staff.
  • Prioritising the weekly MDT meeting, ensuring the quality of the meeting is maintained
  • Dealing with and investigating complaints
  • Act up in the absence of the Lead

    We are looking for an RMN who is highly motivated, innovative, enthusiastic and passionate about service user person centred care to work within the Redbridge Memory Service.

    This band 6 post is suitable for a Mental Health Nurse familiar in the assessment and review of people with memory problems and in supporting the delivery of post diagnostic group interventions to patients who have a diagnosis of dementia and to family carers. Redbridge Memory Service is a busy and high functioning service which operates Monday to Friday, 9.00am to 5.00pm.

    Redbridge Memory Service is a Multi-disciplinary Team who provides excellent levels of care to Service Users and their Carers in the community. The successful candidate will have an interest in the needs of older adults and will have responsibility for a defined caseload utilising evidence-based practice and client centred principles in assessing, planning and implementation. He/she will have excellent verbal and written communication and IT skills.

    We are looking for candidates who are registered Mental Health Nurses, who have an interest in working with older people, enjoy working in a team and are able to contribute positively to the teams continuous improvement culture.

    Person Specification

    Exeperience

    Essential

    • At least 1 year experience working in a healthcare service
    • Must have mentorship training
    • Memory service experience
    • Registered Mental Nurse with at least 1 years experience


    Knowledge

    Essential

    • An awareness of NHS Plan, NICE, NSF and clinical governance priorities
    • Knowledge of medications used in treatment of dementia and BPSD
    • Knowledge of dementia and relevant therapeutic interventions


    Skills

    Essential

    • Basic awareness of IT and IT Skills

    • To be aware and demonstrate the Trust Values
