Applications are invited from suitably qualified mental health nurses with the experience, skills and competencies to undertake the duties and responsibilities of the above post in the Mental Health Network. The post is based within Mental Health Services in West Lancashire in the Home Treatment Team in Bickerstaffe House, Ormskirk and District General Hospital.
The service offers a multidisciplinary approach to provide assessment, risk assessment and a range of interventions to meet the needs of people with complex mental health issues who require an alternative to Inpatient Admission.
This post provides the opportunity for a forward thinking and experienced individual to develop and implement safe, sound and supportive services that embrace the provision of holistic care. With a strong emphasis on multidisciplinary working and the principles laid out in the Recovery Model, you will need to be able to form effective relationships.You must be a registered mental health nurse with excellent clinical skills. You will also need to have a working knowledge of the care programme approach, risk assessment and management, and the ability to explore issues relating to the impact of acute health and complex social needs on mental health and well-being.
Main duties of the job
Ensuring and effective efficient service is provided to all service users referred. To undertake clear and effective assessment, formulation, planning and monitoring of those referred to the Access and Treatment Team and under home treatment. To signpost appropriately.
The Home Treatment Team role is to undertake the assessment of those referred in acute mental health crisis, provide a gatekeeping assessment of those referred for admission to an acute inpatient ward and to arrange home treatment as an alternate to this in order to promote recovery in the home environment.
To work as part of the multi-disciplinary Team to provide a holistic continual assessment and recovery approach in the home treatment service.
