**Fixed term contract / secondment opportunity for 12 months**
An exciting opportunity has arisen for a Registered Nurse to join our fantastic Integrated Discharge Team as a Specialist Nurse dealing with diverse discharges, working collaboratively with external partners, a particular interest in Fast Tracks and D2A3 assessments is desirable.
This post is a 12-month secondment and an excellent development opportunity to gain new skills around discharge, Fast Tracks and D2A3 assessments.
This is a great opportunity for either an existing band 6 nurse or an experienced band 5 nurse to join the wider Integrated Discharge Team and learn a whole new set of skills.
Working as part of the Integrated Discharge Team you will be responsible for identifying and resolving discharges issues, supporting the implementation of discharge to assess, completing Fast Track and D2A3 assessments.
After a period of comprehensive induction, you will work with support of members of our own teams and external partners to achieve not only timely but also appropriate safe discharges.
As the age demography of Norfolk is weighted towards the elderly, many of our discharges will involve liaising with local care homes and community teams to ensure our patients can get ongoing support on the appropriate discharge pathway.
You will also have patients who require complex support to facilitate their safe discharge. This will entail engagement and coordination with multiple external agencies.
Main duties of the job
We are looking for an experienced nurse with a level-head who is aware of the operational pressures of a large acute hospital.
Communication is key. We are looking for someone who can in a professional manner effectively communicate with a wide spectrum of people to achieve the best outcome.
We are also looking for someone who is confident, who, with previous experience/ skills and the bespoke induction delivered, be able to lead multidisciplinary team (MDT) and influence outcomes whilst at all times maintaining a good working relationship.
We are patient-centred and our goal is to deliver quality, timely and safe discharge
In addition, we want to appoint someone who has experience either in current or previous roles in managing expectations and resolving conflict and can demonstrate this ability.
Excellent organisational and problem-solving skills are a must for this role as you will be working with a caseload of patients who each require individual planning which will involve any number of external partners.
Most importantly and in keeping with our Trust's PRIDE values, we are looking for someone who has exceptional people skills as this role requires problem solving with MDT and system partners whilst having to maintain professional relationships.
