An exciting opportunity to join the team and Clinical Co-ordination Centre for Band 6 Clinical Co-ordinator

Full time

We are looking for a registered nurse who are compassionate, motivated and approachable. In return you will be working within a supportive team with strong commitment to excellent customer care.

The post holder will be required to work at both Caludon Centre and St Michael's Hospital to provide an interface between the Clinical Co-ordination Centre and other internal CWPT services and all External agencies.

The post holder will be required to work in a flexible manner working across other main hospital sites within CWPT Acute Mental HealthServices. The post holder will be required to work as a Bleep holder as part of a rota if required, this will be a regular requirement during nightshifts.

The post holder will be required to undertake the Place of Safety assessments as and when required .

Liaise and secure the appropriate bed for informal and detained patients based on clinical risk

Ensure safe staffing is managed across all hospital sites and deploy staff as required to ensure safety is maintained. Escalate any safe staffing reds to the Pathway Manager in line with Trust process

Ensure bed data is received and updated on a regular basis to ensure data is accurate and available to support clinical decision making

Report to a senior member of the team as required and work within professional codes

Provide accurate and timely information relating to the CWPT Bed Capacity

Coordinate all admissions and transfers across the Trust

Monitor care and treatment for patients within the Place of Safety reporting back to multi- professional staff as appropriate

Record patient information on Care Notes as required within Trust Policies and procedures

Conduct risk assessments and maintain a safe environment

Engage with carers and families responding to identified needs

Adhere to and report on safety and quality (governance) issues

Mentor students as appropriate to qualifications and profession

The post holder will adhere to all organisational systems and processes

The post holder will organise their own work load under the supervision of a senior team member. The post holder will contribute to service development

  • Demonstrates a comprehensive knowledge of nursing assessment and care.
  • Demonstrates competence and sound decision-making in the assessment of acutely ill patients
  • Expert competence in the clinical skills required within the co-ordinator role
  • Must be able to contribute to supervisory process for junior staff and learners
  • Must demonstrate a high standard of written and verbal communications skills
  • Able to use computer systems to access and retrieve formation


Experience

  • Must have an extensive experience in an acute hospital environment
