'Better Lives'is the community substance misuse service in Islington: Camden and Islington trust are the lead providers of this service in partnership with Humankind and WDP. We offer treatment to any resident in the borough who has substance misuse issues. Our treatment offers include assessment, care planning and risk assessment , opiate substitute prescribing, psychosocial interventions, one to one key working, hared care support, groups, aftercare support, care management support BBV screening and support from family services ETE and peer mentoring. As a Band 6 nurse we would expect the applicant to have experience working within a substance misuse or mental health setting and have a good understanding of supporting service users in their recovery and especially for those who have an interest in supporting service users with complex needs with their physical health ensuring that they are signposted to the relevant services to improve health outcomes . provide expert, autonomous and highly specialist clinical practice within a multi disciplinary team to include:-

Initial assessment and formulation using a range of mental health, physical health or other specialist tools.

When required to do so making appropriate judgements about own caseload

To assess, plan, implement and evaluate care in negotiation with service users, carers and other services, considering the needs of a diverse community.

Apply a range of therapeutic modalities, using evidence based interventions in accordance with the requirements of service line care pathways.

Job responsibilities

As a nurse, the post holder will be responsible for ensuring that the wider team understands and operates to the requirements of the service operational policy and strategy, and that safe, evidence based, effective, culturally competent care is delivered in a timely manner to service users. They will contribute to governance and audit within the team, ensuring that team members are supported and developed to their potential and that new team members are comprehensively inducted and successfully integrated into the team.

The focus of the role will be that of individual assessment, intervention and evaluation of treatment, delivered within the framework of NICE guidelines, evidence based best practice and to maximise service users recovery and re-enablement ensuring that service users are offered all treatment offers within the Better Lives model across different sites and community settings.

They will be responsible for a defined caseload and they will formulate appropriate specific, highly specialist care and treatment plans providing highly skilled and specialist interventions and care to service users with complex mental and physical health problems.

Essential

• Current registration with relevant professional body.
• Evidence of recent and or on-going post registration academic study relevant to practice

Desirable

• Mentorship or recognised teaching and assessing certificate or willingness to undertake

Knowledge / Experience

Essential

• Significant clinical experience in substance misuse service delivery
• Demonstrable understanding and experience of utilising a variety of assessment tools as well as service specialist tools currently used in practice in relation to assessing / planning care needs and determining risk
• Demonstrate an understanding of the physical health needs within a population with high health inequalities and possible barriers to accessing medial treatment

Skills / Abilities

Essential

• Ability to communicate both in verbal and written form at a high level in a range of complex areas
• Ability to work in an integrated service model, embracing change and contributing to positive outcomes for service users and the service.
• Ability to manage own caseload of service users with complex needs.

Desirable

• Ability to take on supervision responsibility for junior staff.
