A fantastic opportunity has arisen to join a well-established, experienced and passionate team, committed to making a difference in people's lives. The Community Neuro Rehabilitation Team (CNRT) is a multidisciplinary team providing services across the county of Shropshire, including Telford & Wrekin, for people with acquired brain injuries and progressive neurological conditions.
The team has established links with key stakeholders, both within the county and the wider Midlands network, and comprises of registered Physiotherapy, Occupational Therapy, Speech and Language Therapy and Psychology staff in addition to our Support Workers.
The successful applicant will demonstrate excellent communication skills, an ability to work flexibly within the team and be an open and positive individual who wishes to strive to see the best for both our patients and staff. A good understanding of data and the importance of evidence to underpin decision making will be essential to deliver quality and performance change.

Main duties of the job
To work as a key member of the multidisciplinary team (MDT) that is responsive to the needs of the local population, working flexibly to respond to the changing needs of the patient.
To complete Occupational Therapy assessments of patients with diverse presentations, with complex physical and psychological diagnoses, to inform treatment planning based around agreed and realistic goals.
To provide a high standard of Occupational Therapy care and work as an autonomous practitioner, taking responsibility for the management of patients, using robust clinical reasoning to inform care
To hand over care delivery as appropriate to rehabilitation assistants, and carers. To review this with an agreed timescale.
