The post holder is responsible for the day to day operational leadership of the Integrated Discharge Team, providing direct line management to healthcare professionals employed directly by SASH (Discharge Team) and working collaboratively with the multi-agency senior management team to lead colleagues from Surrey and Sussex and Health and Social Carewho make up the Integrated Discharge Team.

The post holder will provide senior clinical oversight and input to care planning for patients medically ready for discharge, who are being cared for in designated areas within the hospital, and drive discharge planning aimed to meet their needs, reduce length of hospital stay and improve customer experience.
