A rare opportunity has arisen for a Advanced Nurse Practitioner in our dedicated older people's community mental health team.
The multidisciplinary teams comprise of Nurses, Occupational Therapists, Support workers, physiotherapist, assistant psychologists, Psychologists and Doctors. We are committed to the excellent community care of older people with a breadth of mental health problems and those with dementia.
To assist in the continued delivery and development of high-quality community nursing services, working towards being an Advanced Practitioner, maintaining patterns of excellence and ensuring that service delivery meets the needs of patients within the care home/MATS communities in Bradford/Airedale district. To ensure communication and support is maintained for service users by linking in with the inpatient services to ensure seamless care provision. Care Home Liaison/MATS is a service that is being reviewed at current times and would provide the successful applicant with the opportunity to shape how the service moves forward to continue to meet the needs of this client group.

Main duties of the job
Continue to develop own knowledge and skills to meet NMC standards and ensure competent practice to meet the roles and responsibilities of the post.
To ensure equality of opportunity and fair access for all client and carers of service
Use best available evidence to inform and develop personal and team clinical practice.
Construct, implement and evaluate packages of care to effectively meet the needs of clients and their carers in accordance with the current guidelines for the care programme approach.
Clinical Assessment and risk Assessment- this to be reviewed with senior practitioners
Undertake accurate, timely and skilled assessments and ongoing reassessment of patient's condition, taking into consideration all social, physical and environmental factors analysing a range of options
Comprehensively and accurately record and evaluate the implications of the information gained in order to plan the appropriate care for the patient.
Detect early signs of deteriorating mental health including potential self-harm and suicide risk, worsening physical conditions and potential threat to others.
Effectively manage direct exposure to sometimes unpleasant working conditions, e.g., bodily fluids, unhygienic environments and managing challenging behaviour and verbal aggression
To prescribe medication once qualified within nice guidance, NMC guidance and Local trust policies
