We are excited to advertise a Community Matron role to work collaboratively within our Integrated Community Team across the different streams delivered by our award-winning services of Hospital at Home, Case Management, and Core Nursing.

We deliver high quality nursing care across the Welwyn and Hatfield locality to prevent admission to hospital, facilitate discharge, provide case management to people living with long term conditions and to support District Nurses to deliver high quality planned nursing interventions to people with long term conditions.

Hertfordshire Community Trust has a Good CQC rating acquired in May 2020 and we are working towards achieving an Outstanding rating.

The successful candidate would be supported to develop skills in assessment, planning care, intervention, and evaluation of people with complex needs and will be joining a team who are committed to demonstrating the trust values who care about each other as well as their patients.

We work collaboratively with many partnership organisations across East and North Hertfordshire including Integrated Care Board, Hertfordshire Foundation Trust and Hertfordshire County Council.

Main duties of the job

The post-holder will be an excellent communicator and work well both individually and within a team. You will be patient focused and have the ability to undertake clinical assessments to arrive at a differential diagnosis, start, plan and evaluate care. You will also have organisational and time management skills and be able to demonstrate leadership skills across the inter-professional team.

To work as part of the Integrated Community Care team to prevent inappropriate hospital admission and have the capability to provide assessment and treatment of patients.

To deliver a high standard of patient care using evidence based framework within East & North Hertfordshire.The post holder will work autonomously to provide a comprehensive,advanced assessment and care package to the patients and families, utilizing support services and liaising with other agencies to ensure the complex needs of this client group are met.To provide advanced nursing assessment, diagnostic, prescribing, care and treatment, marshalling resources, overseeing the quality of care from all agencies and co-coordinating acute, primary and social care ensuring that the focus of care remains at home and in the community where possible.

Job description

Job responsibilities

This post carries continuing responsibility for pro-active management of a caseload consisting of identifiedhigh intensity users with complex needs. The post holder will work autonomously to provide a comprehensive,advanced assessment and care package to the patients and families, utilizing support services and liaisingwith other agencies to ensure the complex needs of this client group are met.To provide advanced nursing assessment, diagnostic, prescribing, care and treatment, casemanagement and anticipatory care for patients with complex LTCs, marshalling resources, overseeingthe quality of care from all agencies and co-coordinating acute, primary and social care ensuring that thefocus of care remains at home and in the community where possible.

Able to support Hospital @ Home, using advanced skills to prevent hospital admission and to ensure service users receive hospital level care within their own homes.

The post holder will work locally with ICT core team sharing their advanced knowledge with the team, facilitating joint visits dependant on the demand of the caseload.

Person Specification

Qualifications and Training

Essential

  • RN First Level Nurse/AHP.
  • Community Nursing Qualification or equivalent post registration advanced AHP qualification.
  • Specialist Practitioner or equivalent AHP.
  • Independent Nurse Prescriber - mandatory to pass and practice.
  • Demonstrates a high level of relevant clinical experience.


Experience and Knowledge

Essential

  • Extensive experience working with older people.
  • Continuing professional knowledge and development.
  • Experience of teaching and mentorship.
  • Change management and leadership skills.


Skills and Abilities

Essential

  • Evidence of ability to communicate both verbally and in writing at all levels in multi-disciplinary forum and with users and carers.
  • Demonstrate competence in 'top to toe' assessment
  • Demonstrate competence in safe medicines prescribing
  • Demonstrate competence in phlebotomy
  • Demonstrate competence in administration of intravenous therapy

  • Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
