To work with the multi-disciplinary team to facilitate the transition from children's to adultsservicesTo engage with, educate and empower young people to make decisions and manage theirtreatment regimes.To act as an advocate for the young person and their family, giving expert support and advicebased on best practice to promote self-care through appropriate strategies.
