We are looking for a dynamic, enthusiastic nurse to engage, educate and empower young people to make decisions to manage their treatment.

Main duties of the job

To work with the multi-disciplinary team to facilitate the transition from children's to adultsservicesTo engage with, educate and empower young people to make decisions and manage theirtreatment regimes.To act as an advocate for the young person and their family, giving expert support and advicebased on best practice to promote self-care through appropriate strategies. To provide teaching sessions in the clinical area and appropriate student and staff trainingmodules.

Job description

Job responsibilities

To work with professional groups and other agencies in both primary and secondaryhealthcare settings and attend relevant meetings.To develop transition policies, protocols, guidelines and systems of care in collaboration withthe multidisciplinary team.

To act as an expert resource to colleagues, members of the multidisciplinary team in theward, outpatients and community.To maintain a high visible profile in the clinical area, acting as a positive role model.To promote the service across the Frimley Integrated Care System

  • Current NMC registrant
  • ABLE TO WORK AUTONOMOUSLY
  • PREVIOUS EXPERIENCE OF TEACHING
  • ABILITY TO INITIATE, SUSTAIN AND EVALUATE CHANGE
  • EVIDENC EOF CONTINUING PROFESSIONAL DEVELOPMENT


Desirable

  • PREVIOUS EXPERIENCE IN AUDIT
  • EXPERIENCE IN MANAGING CHANGE
  • PARTICIPATING IN NURSE DELIVERED CLINICS
