Are you looking for a new challenge where you can make a difference in the lives of vulnerable young people aged 10-18 with offending behaviour? To assist young people who are also at risk of exploitation, County Lines, substance misuse and have complex needs, to overcome the obstacles they face and to achieve their potential.
We have an exciting opportunity for a Nurse, Occupational Therapist or Social Worker to join Surrey & Borders NHS Foundation Trust (SABP) in the newly transformed Reaching Out service as part of the Assertive Engagement Pathway. Reaching Out service focuses on engaging and supporting the most isolated and vulnerable children and young people through a multi-agency network.
We are looking for clinicians who are creative, highly motivated, show initiative and are passionate and committed for ensuring high quality services for people with mental health needs.
There are 2 full-time posts covering the county of Surrey with SABP bases across the county. This role requires travel between multiple sites during the working day.
Main duties of the job
To manage a caseload of young people referred to the service, working collaboratively with the Youth Offending Team to provide consultations and mental health assessments for young people.
To build a trusting relationship to develop a deeper understanding of the needs of young people and be able to signpost or refer to more appropriate services. This role will involve outreach into the community to meet young people in their own or choice of environment.
To work in partnership with young people, families/ carers and the Youth Offending Team regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions.
