An exciting opportunity has arisen for a passionate registered learning disability nurse to join the Promoting Access to Mainstream Health Services team (PAHMS). This will be in the capacity of a lead nurse band 7 for our Wolverhampton team but may include working across the Black Country as part of the wider PAMHS team. This post will allow the successful candidate to play an active role in reducing health inequalities for individuals. It will involve working both strategically and having direct patient contact in making a real positive difference to patient experience. We are looking for someone with strong leadership skills and an excellent understanding of the support needs of people with learning disabilities.

The PAMHS team have a responsibility for supporting the development, monitoring and auditing of initiatives designed to reduce health inequalities at both strategic and individual levels, with a strong focus on achieving positive health outcomes for adults with learning disabilities, working collaboratively with community health partners.

Main duties of the job

  • To display, promote and monitor high standards of organisational skills, planning and priority setting both clinically and organisationally.
  • Providing learning disability awareness training to outside agencies, including sharing information on learning disability services and the support available
  • Providing advice on reasonable adjustments to primary and secondary healthcare services.
  • Ensuring that the learning disability population is included in local Clinical Commissioning Groups (CCG) and Public Health (PH) agendas and complementing the work of local health promotion teams
  • Ensuring that national screening and vaccination programmes consider the needs of people with a learning disability
  • Developing inclusive care pathways with generic services that meet the needs of people with a learning disability
  • Monitor team case load sizes and ensure effective outcomes

The PAMHS teams will venture to empower individuals to take greater control over their own health and to be active participants in decision making about their health care.

Job responsibilities

  • To be accountable to the Service Manager.
  • To work collaboratively with members of the wider PAMHS team, in order to provide personalised and appropriate care and support to people with learning disabilities to access mainstream healthcare.
  • To ensure effective liaison between other services to ensure a multi-disciplinary team approach.
  • To be responsible for implementing service development, resource allocation and policy development.
  • Developing inclusive care pathways with generic services that meet the needs of people with a learning disability.
  • Supporting the LeDeR agenda by reviewing the deaths of adults with learning disability and assisting in the drive of local service improvements based on themes emerging from LeDeR reviews at a regional level.
  • To be responsible for the supervision and appraisal of team members.
  • To ensure clinical care is evaluated for effectiveness and promotes optimum health and well-being and meets agreed outcome measures.
  • To be part of a multi-professional community team providing specialist clinical learning disability nursing services for people over 18 years of age enabling access to mainstream primary and secondary care.
  • To teach and mentor nursing students.
