Are you passionate about enabling change in patients who misuse substances?
An exciting and fulfilling job opportunity has arisen within the Substance Misuse Team at HMP Ranby.
Working with a caseload of often complex and challenging patients you willprovide a substance misuse through-care service for patients from the point of entry into the prison system to re-entry into the community so that re-offending is reduced and the health and social functioning of patients is improved.
You will be part of a committed, passionate and vibrant Substance Misuse Team delivering high quality patient care through assessing, planning, implementing and evaluating care for people with substance misuse problems, working to harm reduction and health promotion / recovery agendas.
You willprovide leadership to junior staff by maintaining professional standards, ensuring adherence to all relevant policies and procedures to ensure the delivery of high quality care.
Main duties of the job
To assist in the provision of effective substance misuse health services for prisoners which deliver against required standards, e.g. Key Performance Indicators, Prison Health Performance Indicators and demonstrate value for money, improve the health of individuals, enabling the service to compete as provider of choice to local commissioners.

The Substance Misuse Team is part of the Healthcare Department which also provides Mental Health and Physical Health services, all of which work together to provide a comprehensive package of patient care.
The Offender Health Directorate provides healthcare to seven Prisons and one Immigration Removal Centre in the counties of Lincolnshire, Leicestershire and Nottinghamshire. As such you are part of a large peer group of 400+ healthcare professionals with access to appropriate peer support, supervision and excellent opportunities for learning and development.
Applicants should be enthusiastic, motivated, patient focused and enjoy a challenge. The Psychosocial Service is predominantly a Monday to Friday service, however staff will be required to cover the service over a 7 day period to accommodate service needs as required. The team operates in a supportive environment with a commitment to personal and professional development.

Job responsibilities
- To carry a complex psychosocial case load with responsibility for assessing, planning, implementing and evaluating patient care.
- To undertake comprehensive assessment of patients needs following referral to the service.
- To plan programmes of psycho-social interventions using evidence based practice
- To provide information and advice about drugs, alcohol, smoking cessation and related issues, to include blood borne viruses and safer sex.
- To co-facilitate group work relevant to substance misuse and offending behaviour.
- To ensure care plans are formulated acted upon and reviewed.
- To effectively liaise and develop links with services within the prison setting and in the community to establish effective through care and after care pathways.
- To provide smoking cessation interventions in line with national guidance and locally agreed quit targets.
- To act as a second check and signatory in the dispensing of controlled drugs in relation to substance misuse.
- To participate in prison induction programme.
- To effectively plan discharges and prison releases in conjunction with community services
- To maintain accurate healthcare records in line with Trust policy
- To attend and actively participate in team meetings, helping to promote
