To be responsible for providing high standards of holistic nursing to all lung cancer patients and their family/carers across East Lancashire NHS Trust ensuring ongoing provision of support, education and guidance
To work autonomously as well as part of the wider Multi-Disciplinary Team (MDT) being accountable for own professional practice.
Work as a resourceful member of the HPB & UGI Cancer Clinical Nurse Specialist (CNS) team supporting the HPB & UGI Cancer CNS's in ongoing service development, liaising effectively with all members of the multidisciplinary team.
Main duties of the job
Act as patient advocate to ensure the delivery of care is focused on the HPB & UGI cancer patient and family
Support relevant clinical l and non-clinical staff across hospital wards and departments and community care settings
Proactively support the wider MDT with planning, implementing and evaluating care
Support in the tracking of patients to ensure timely access to agreed treatment plans ensuring organisational compliance and achievements of cancer performance targets
Support in the developments and delivery of nurse led risk stratified follow up and patient Supported Self-Management
Work closely and collaboratively with other teams within cancer services.
Encourage patients and carers to be involved in decision making for their care planning and treatment choices.
Be able to communicate sensitively with patients/ carers, giving information, offering support, empathy and reassurance regarding their on-going condition.
Communicate very sensitively with often very distressing information for patients and their family/carers.
To maintain precise nursing documentation relating to patient care and ensure that all clinical and legal documents completed are both accurate and legible and maintain confidentiality at all times.
Be able to communicate in demanding and often challenging situations.
Provide professional clinical advice either via the telephone or in person to other colleagues both internal and external to the organisation.

Ensure all patients have Holistic Needs Assessments and Treatment Summaries at all key stages of their clinical pathway.
Participate in and undertake clinical audit to aid service development.
Take responsibility for own professional growth and keep up to date with professional developments and research.
Participate in the development and implementation of clinical guidelines and policies with other members of the team and review them on a regular basis.
Support patients within HPB & UGI cancer trials pathways.

Represent the HPB & UGI CNS at meetings and be prepared to take the lead in terms of preparing and presenting meetings.
Support inpatient pre & post operative on the surgical wards
