An exciting opportunity has arisen for a passionate and enthusiastic Mental Health Practitioner to work within the Bury Community Mental Health Team. The successful candidate will be joining a dynamic Multi-Disciplinary Team at an exciting time of positive change.

The post holder will be expected to demonstrate high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems. The role will require clinically effective assessment, care management skills and interventions in accordance with the Care Programme Approach and Care Act framework.

The post holders duties will include engagement with clients in the development of person centred, recovery focused care plans, supporting our experienced duty team with short-term pieces of work and supporting people managing crisis. You will need to demonstrate excellent communication skills and be able to work collaboratively in a multidisciplinary setting. You will be dedicated to delivery, both as part of a team and working independently, of a high-quality, innovative, recovery focused and person centred care which meets the needs of people who use our service and their carers.

Main duties of the job

The successful applicant will work effectively with adult service users with mental health needs across Bury. The post holder will be highly skilled and competent in both holistic health assessment and identifying social needs, risk management and Formulation, developing collaborative care plans, facilitating the implementation of the agreed plan for care with a focus on recovery and social inclusion. The post holder will provide a range of interventions and the opportunity to develop these within the role.

Job description

Job responsibilities

-The Practitioner will provide comprehensive mental health service to adults of working age who experience moderate to severe mental health problems in a community setting.

-Utilise the Care Programme Approach to support and promote the recovery of service-users under the care of the service.

-Working collaboratively across a range of disciplines to continuously promote a needs based, recovery focused approach to all service-users.

-Work within, and promote, an integrated approach to service delivery.

-Develop clear relapse recognition/prevention plans in a collaborative manner and share with MDT, service-users and carers where appropriate.

-Develop meaningful engagement with service-users and carers.

-Offer support as needed as part of Duty Rota.

-The Team offer a Monday-Friday service within the hours of 9am-5pm.

-Flexible working can be discussed in line with Trust Policy.

Person Specification



-Registered Mental Health Nurse or allied health professional (Occupational Therapist/Social Worker).

-Evidence of continued professional development.


-Short courses or experience up to post-graduate diploma level.

-Mentorship qualification (or equivalent) or willingness to compete training

-Psychosocial/CBT Qualification or experience in delivering these interventions.

-Non-medical Prescriber



-Experience of working within a Mental Health setting as a qualified member of staff

-Experience of developing collaborative care plans and risk assessments

-Experience of working within a Multi-Disciplinary Team

-Experience of working with people with a range of complex mental health problems


-Experience of working within a Community Mental Health Setting

-Experience of clinical supervision of other staff



-Knowledge of relevant legislation underpinning role Mental Health Act, Care Act, Mental Capacity Act

-Understanding of working within an Integrated Team

-Comprehensive understanding of Safeguarding adults procedures and practice.

-Awareness and understanding of Care Programme Approach

-Working knowledge of therapeutic interventions and assessments for people with complex mental health needs

-Awareness of key national and local policies and drivers in relation to adult mental health services


-Psychosocial Interventions

-Knowledge of CCG and Local Authority commissioning processes



-Ability to undertake the role of a Care Co-ordinator

-Evidence of ability to provide specialist mental health assessment and management/assessment of risk and demonstrate sound clinical judgement.

-Excellent written and interpersonal communication skills.

-Ability to manage own emotions and deal with others in stressful situations.

-Ability to engage with, and provided therapeutic interventions too, client group and carers.

-Ability to prepare, record and complete relevant reports to a professional standard.

-Ability to manage a diverse and complex caseload and prioritise own time and work under pressure.

-Demonstrating and valuing diversity and respond constructively to discriminatory behaviour.

-Ability to use I.T systems efficiently.

Work Related Circumstances


-Use of a vehicle or access to means of mobility to travel across Trust footprint in line with service needs.
