37.5 hours per week. We are seeking a Band 5 Registered Nurse to join the Community Nursing Team in the West Devon Neighbourhood covering Tavistock and surrounding areas served by the West Devon Primary Care Network. This post is apermanent post in a team covering 7 days a week 8.30 17.00 so rostered weekend working will be required.

You will work autonomously within a team based in rural practice, to provide high quality holistic nursing care designed to meet their individual needs, maximizing well-being improving health outcomes and supporting people where possible to remain at home. You will need to demonstrate initiative with sound clinical knowledge, judgment, and evidence based practice.

We are currently developing our existing services to deliver urgent and acute care at home which means an opportunity to have additional competencies in your skillset that now includes intravenous therapy. We are in the process of enhancing our frailty services to prevent unnecessary hospital admissions and we also have a keen focus on supporting those caring for someone with dementia. We are looking for a Community Registered Nurses who are passionate about providing high quality, competent care to people in their own homes. To promote a whole service, in a coordinated way and improve patients experiences. We are looking for people who are flexible and excited about being part of a transformational service which aims to build care around the needs of the community and explore hybrid nursing roles to bridge the gap between home and hospital.

You will be part of a Team and managed by a District Nurse Team Manager.

The role requires positivity, excellent communication, customer care and organisational skills, good time management and the ability to prioritise own workload.

Reports to (Line Manager):

Community Sister/Charge Nurse (referred to as Caseload Holders within this job description) and/or District Nursing Manager.

Accountable to (Professionally/managerially): (optional)

Director of Professional Practice, Quality and Safety Locality and Deputy Locality Managers.

Dimensions and Context of Role:

7.1 The post holder works as a member of the District Nursing and Primary Health Care Team during their working day.

7.2. The post holder is supported by the Caseload Holder and is responsible for providing skilled nursing care and information to patients and carers.

7.3. The aim of which is maintaining patients in their own homes and enabling them to enjoy maximum independence and optimum quality of life.

7.4. The post holder will have a clinical role within the District Nursing Team they will have responsibility for admitting and discharging patients to and from the caseload(s).

7.5. The post holder is responsible for assessing, triaging, planning, evaluating and implementing care which may be complex, sensitive and unpredictable with the support of a Caseload Holder.

7.6. The post holder is responsible for coordinating the caseload in the absence of the Caseload Holder on a temporary basis. This will be reviewed after a three-month period by the line manager or a Manager.

7.7. The post holder will work alongside Health and Social Care Services within statutory and voluntary sectors providing appropriate nursing skills to ensure care packages are responsive to individual needs of patients within their own homes and other care settings.

7.8. To signpost to appropriate services, statutory and voluntary bodies.

7.9. To undertake and to develop all nursing skills and competencies, updating as required for the role to enhance the skill mix of the nursing team, ensuring that the ever-changing health needs of patients are met.
