Multi-Professional Advanced Clinical Practitioner
(Open to HCPC / NMC Registered Professionals)
We continue to grow as a recognised and respected multi-professional frailty and urgent care response service, and we are excited to be advertising our last Multi-Professional Advanced Clinical Practitioner (ACP) position. The successful candidate will join an established ACP team with a wealth of experience and knowledge in frailty / urgent care delivery and be supported by the Nurse Consultant, Chief of service, Consultant Geriatricians, Visiting GP's and wider directorate senior management team.
Managing patients with frailty is a complex business. It requires a diverse knowledge base and experience. We are looking for an established and experienced Multi-professional ACP who has worked within a variety of clinical settings and speciality's, who can demonstrate a broad breath of clinical acumen, autonomy and professional standards. In addition transformational service change initiatives and the leadership qualities the team will wish to follow. You will also be experienced and comfortable in navigating uncertain clinical and strategic terrain and be solution focused.
Whilst this advert is for a qualified ACP we do not wish to miss the opportunity of a nearly qualified to apply, who can demonstrate the breath of clinical knowledge for this role but also the service transformation experience. Our new nurse consultant is all about finding that 'something different'. The team and its leadership is very forward thinking and the team will be expanding so there will be plenty of opportunity for career development and coaching.

Main duties of the job
To work collaboratively with the Consultant Nurse, Consultant Geriatricians, Head of Nursing, Lead Nurses, Head of Therapies, Matrons and external partners to further develop Frailty pathways, including the acute Frailty service in ED, inpatient frailty liaison.
To provide high level clinical acumen and advice to other professionals / patients / relatives
Practice autonomously, whilst being accountable and self-directed in-line with their relevant code of professional practice.
To undertake Comprehensive specialist holistic assessments for individuals presenting with frailty within the acute care setting, with the purpose of creating an individualised agreed management plan incorporating pharmacological considerations, long-term conditions management, frailty syndrome identification, prevention and management, diagnostic reasoning and initiating onward referral to specialist teams as appropriate.
To provide expert clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision-making skills.
To lead as a driver for change, to monitor and improve standards through supervision, evidence-based practice, clinical audit, research and education.
