District Nurse Liaison Band 6 - Permanent - 37.5 hours
The Tameside & Glossop District Nursing Service are currently looking to expand our DN liaison service and have an exciting opportunity for a band 6 Community Specialist Practitioner to join our team.Our service works across the 5 neighbourhoods of Tameside.
The role may require you to work across the localities as a service need requirement.
This is an exciting opportunity to become part of a developing integrated organisation. If you are a self-motivated, dynamic individual with a passion for community care, this could be the position for you.
Main duties of the job
The post holder will have responsibility for management of the routine activities of the District Nursing Triage Team, organising workload and staff rosters. Exercising leadership and judgement to ensure good clinical practices and high standards of care are maintained in order to meet service demand.
The post holder will have responsibility for providing District Nursing expertise and knowledge for the District Nursing Triage team with the support of the District Nurse Liaison Lead ensuring the principles of Community Specialist Practice are embedded in day to day practice.
They will support the workforce to deliver evidenced based wound care, excellent End of Life care provision and support the discharge plans for patients ensuring seamless transition back to the District Nursing Service.
This post will involve working alongside the District Nursing teams to ensure patients admitted to or discharged have continuity of excellent care provision.
