The Outreach Nurse will provide a clinical service to children and their families in line with trust policies ensuring appropriate use of tier IV services and improving transition to recommended and/or back to referring services. This will involve liaising and working with professional colleagues to maximise the mental health service provision of children and their families within the region. The post holder will examine regional service provision and engage in the development of service initiatives which best support provision needs at a local level.
This post will concentrate on supporting and developing the mechanisms for involving parents and patients in the current service provision and any future developments.

Main duties of the job
- To work autonomously in supporting and developing a seamless service of care pre and post discharge across tiers 3 and 4.
- To work with those children and families who are expecting such serious difficulties that they may require inpatient admission.
- To provide support to children and families in their homes and offer therapeutic interventions for those awaiting Croft beds.
- To contain anxieties and engage vulnerable and difficult to engage patients and families with the Croft enabling them to access assessment and treatment programmes.
- To assess, plan and implement outreach care for these children and families autonomously.
- To be competent and clear regarding assessment of risk and relevance of this when offering professional opinions in an assessment.
- To take part in the planning and evaluation of care of patients at review meetings, case conferences, team meetings, ward rounds etc.
- To remain engaged with developing relations with parents during their inpatient stay.
- Able to undertake Physical Intervention training provide by the Trust
- Valid full UK Driving Licence required as part of this role can include travelling locally and long distance to visit families in their homes and to support patients to visits their home and school.

- To be key in the development of the provision of the outreach service in conjunction with the Croft steering group and the maintaining of ongoing review and evaluation of the service provision.
- Manage own caseload and be accountable to Clinical Nurse Specialist/ Ward Manager.
- To supervise staff as required ensuring maintenance of good practice and adherence to working practices of the outreach team, inpatient team and the trust.
- To be responsible for receiving supervision and to use it appropriately.
- To provide leadership and manage junior staff members in the absence of senior staff.
- To comply with the trusts health and safety guidelines in providing a safe environment for staff, patients and their families.
- To comply with the trusts policies and procedures reporting problems to senior staff and taking appropriate action if circumstances arise which could be detrimental to safe standards of practice.

- Professional qualification e.g. First Level Registered Nurse (Mental Health)
Desirable
- Post graduate qualification in a relevant therapeutic intervention e.g. family work, individual work with children, parenting work.
Experience

- Extensive experience of working in child/adolescent mental health at Band 5 or equivilant
- Experience of working within a multi-disciplinary team within a CAMH service
- Child protection experience and trianing
- Experience in family work

- Experience of working in a residential setting with children
- Experience of individual work with under 12's
- Experience and training in managing challenging and violent behaviour
