Dorset Healthcare are looking for Community Specialist Nurses to join our Intermediate Care teams based across Dorset.

The multi-disciplinary team work with adult patients in their own homes, Community Hospital Wards or Care homes to promote independence and improve quality of life by supporting them to remain at home. The role of Community Specialist Nurse provides specialist assessment, advice and interventions to avoid unnecessary admissions to hospital and support timely discharges from Hospital. The role also supports the multidisciplinary team to manage longer term rehabilitation for those with a long-term condition or experiencing a period of physical disability.

Community Specialist Nurse will be required to provide holistic clinical assessment of patients and be able to problem solve and make decisions about their treatment. The post holder will be working as part of a Multi-disciplinary team to support people experiencing reduced function and independence due to a deterioration in health to remain at home or return home as quickly as possible. The role will also provide follow-up monitoring and assessment of these patients on your own caseload.

This role is a full time post however part time would also be considered as long as the services needs can be met. Hours covered by the service are 08:00-20:00 7 days per week with weekend enhanced pay!

Main duties of the job

Dorset Healthcare are expanding our Intermediate Care Services through development of virtual wards to enable people whose health needs would ordinarily be managed in hospital, be managed at home. This role will play a pivotal part in the development and support of this service.

We offer opportunities for further training and development. There is also the chance to be involved in Service development projects.

Undertake specialist autonomous assessments, interventions, and management of patients on an independent caseload and as part of the wider multidisciplinary team.

Provide specialist advice to others regarding the management and care of patients / service users.

Responding to patients in crisis in their own homes to avoid unnecessary hospital admission.

Providing follow-up and monitoring through clinical assessment and interventions to support people to return and remain at home.

Undertake clinical observations and holistic assessments, acting on findings and liaising with the MDT as required.

Devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

Provide a high quality person centred approach to care delivery which always considers people's safety, privacy and dignity.

Person Specification

KNOWLEDGE, SKILLS AND TRAINING

Essential

  • Registered practitioner to degree/diploma level supplemented by post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty
  • Membership of the relevant Professional Body
  • Learning and Assessing in Practice Qualification or equivalent practice assessors training
  • Evidence of recent professional development in an up to date portfolio


Desirable

  • Knowledge and understanding of Trust Strategy relevant to role


JOB SPECIFIC EXPERIENCE

Essential

  • Experience at Practitioner Band 5 level
  • Experience of specialist working
  • Experience of managing change
  • Demonstrable knowledge of assessment and therapeutic interventions in area of specialism
  • Experience of developing specialist programmes of care for an individual or groups of patients/clients and of providing highly specialist advice
  • Able to demonstrate specialist clinical reasoning skills to assimilate information in order to make a clinical judgement regarding diagnosis and intervention.
  • Ability to prioritise and organise workload effectively


Desirable

  • Recent previous experience within a comparable role


MANAGERIAL/SUPERVISORY EXPERIENCE

Essential

  • Experience of providing clinical supervision and mentoring to junior staff
  • Experience of devising and delivering training

  • Able to effectively manage available resources in the pursuit of quality service provision ensuring a safe environment


INFORMATION TECHNOLOGY/RESOURCES
