The post holder will be expected to deliver care at an expert level of clinical practice within a variety of community settings in the field of frailty and elderly care within the division of Cheshire West Integrated Care Partnership. The higher-level Nurse Consultant will have extensive skills, clinical knowledge and experience with a Masters being essential. Clinical duties will be undertaken without direct supervision but in collaboration with a designated clinical lead. They will need to plan manage and complete episodes of care undertaking comprehensive assessments and management along with complex clinical decisions regarding service user management and clinical outcomes.The higher-level Nurse Consultant will practice in conjunction with the multidisciplinary team with their decision making and clinical level of patient management reflecting that of a middle grade doctor. They will play an integral role in taking forward system changes and leading in the operational and strategic development in all aspects of community frailty services to prevent patients attending and being admitted into the acute hospital. Responsibility and contribution for continuous development of clinical practice, research and standards of care within the community service along with the development of policies, guidelines and pathways to ensure the elderly residents have a seamless pathway with the right care in the right place at the right time.

Main duties of the job

To provide expertise and clinical leadership in the assessment, diagnosis and treatment of patients discharged from the Acute Hospital to the Community Unit and those patients referred to the Community Rapid Response Team, via the Emergency Department, Medical Assessment Unit, Same Day Emergency Care, Community Nursing/Therapy environments. This will include liaison with, as well as challenging or complex negotiations with patients and other agencies in relation to care.It is essential that the postholders practice is underpinned by both the four pillars of Advanced Practice and the NHS Knowledge and Skills Framework and will be expected to operate at level 4 within each of the dimensions.

They will be expected to work collaboratively with other healthcare professionals making appropriate referrals to specialist medical clinicians where necessary. In doing this they will develop and work across professional, organisational and system boundaries improving services and developing practice.The post holder will work throughout the Cheshire West Integrated Care Partnership within all areas of community care including rehabilitation and care home provision. They will be expected to act as a practice leader whilst participating in local, regional and national networks.
