A full-time vacancy have arisen with the Complex Care Team for experienced Registered Nurses (RGN, RMN or RNLD) to undertake the role of Complex Care Nurse Assessor, based in Mamhilad, Pontypool.The role will require the post-holders to undertake in-depth expert / specialist nursing assessments of individual residents in nursing homes and hospital sites across the five Locality areas of Gwent, on behalf of Aneurin Bevan University Health Board, to ascertain individual's eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess and entitlement to NHS Funded Nursing Care (FNC) funding. Consequently, the roles will require the post-holder to facilitate the Multi-disciplinary Team assessment process, and to monitor the individual's changing needs and agreed packages of care within the homes.
Main duties of the job
The post-holders will promote collaborative and inter-agency working across professional and organisational boundaries by providing professional advice, support and guidance to both professional and non-professional colleagues, individual residents and their relatives / representatives.
The ability to speak Welsh is desirable, but not essential for this post Welsh and/or English speakers are equally welcome to apply
