We are seeking highly motivated Specialist Community Public Health Nurses to join our 0-19 Healthy Child Programme in Norfolk

The successful applicants will work as part of a dynamic team leading delivery of our Healthy Child Programme. This will include implementing planned programmes of care and evaluating care delivered.

Liaison with other agencies is also a key part of the role and will include consultations with professionals from other agencies.

The contacts are delivered face to face or by video platform to support the range of issues experienced by children and young people aged 0-19 and their families.

Main duties of the job

As part of the Healthy Child Programme, Family Public Health Nurses work within a defined locality, assessing need, delegating work within the team, implementing planned programmes of care and evaluating care delivered.

The role is to provide a child centred, inclusive, holistic and accessible Public Health programme to children, their families and communities, establishing links with the Early Childhood and Family Service, Early Years Settings, Early Help, other Health providers, communities and agencies (Education, School Services, Health and Voluntary organisations).

To promote the physical and emotional well-being of children, and young people, supporting them to establish good health now and in the future. To promote early intervention, health promotion, advocacy, empowerment, inclusion and the reduction of health inequalities to ensure children are supported to reach their full potential.

Job responsibilities

To work with vulnerable families consisting of children and young people 0-19 years of age to be the one key clinical HCP contact for consistent family support.

To work as part of a multidisciplinary 0-19 HCP team to identify the health needs of children and young people, their families and the community and contribute to developing a strategy to address individual identified needs as well as those needs within the community.

To work with targeted community populations and to delegate work to the skill mix team as appropriate

To use knowledge and skills to assess, plan, implement and evaluate packages of care

To provide in person and video contacts to assess the needs of children and young people (0-19) and their families. These contacts will also be provided and supported by the use of a range of different digital technologies.

To promote the use of digital resources on Just One Norfolk to enhance self-care skills for children, young people, and their families

To confidently utilise virtual platforms in assessments, interventions, training, and networking to support effective practice and partnership working

To contribute to the ongoing development of digital methods to support our service delivery.

To offer clinical supervision to the HCP skill mix who support children and young people and their families.

To provide caseload supervision to the HCP skill mix who will be delivering agreed interventions.

To plan, deliver and evaluate evidence based health promotion community support with the wider 0-19 locality team.

To advise their client group on the promotion and protection of health

To link practice with local and national targets and deliver public health programmes based on current priorities and targets.

To act as a health link for other agencies, e.g. Early Childhood and Family Centres, GPs, schools and early years settings, voluntary services, public and private agencies and work closely with partner agencies to promote the health and wellbeing of children, young people and families

To support children, young people and families who are considered to be vulnerable or at risk. To adhere to current Safeguarding Procedures, and participate in any safeguarding meetings as required.

To contribute to the development of local policies and procedures relevant to their area of work

To take responsibility for their own clinical practice including clinical supervision and to adhere to the NMC code of conduct.

Practitioners based within a locality team may also be required to work within the Single Point of Access as service need requires and to act as a resource.

To contribute to the ongoing development of digital methods to support our service delivery.

Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice. Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport to meet the travel requirements of the role. This is subject to the provisions of the Equality Act 2010, and therefore if you require a reasonable adjustment due to a disability then this will be considered.

Qualifications

Essential

• oRGN/RMN/RSCN/RN - LD/Registered midwife
• oSCPHN qualification
• oEvidence of completion of migratory portfolio to demonstrate dual qualification or willing to work towards

Desirable

• oMentor/998 or equivalent teaching qualification
• oRecognised leadership course
• oV100 Non medical prescribing

Experience

Essential

• oExperience or working with children and their families.
• oExperience of profiling localities and need.
• oExperience of working in the community
• oExperience of working in a multi- disciplinary setting
• oExperience of Safeguarding

Desirable

• oExperience of Mentoring
• o2 years experience as a SCPHN

Skills and Knowledge

Essential

• oHigh level of clinical skills, with the ability to identify, respond to and evaluate health needs
• oCommitment to the development of excellent services, which focus on improved outcomes for children
• oCurrent knowledge of local and national policies informing public health practice
• oAbility to communicate effectively with children, parents and Early Years settings, schools and other health organisations

Desirable

• Standard keyboard skills and ability to communicate through IT using packages such as Word, Outlook, Excel and PowerPoint
• Ability to use Systm One
• Experience with delivering healthcare interventions via digital platforms
• Leadership and line management skills
