An exciting opportunity has arisen for a highly specialist OT/PT to be part of our Rehabilitation team which is part of Westminster CIS (Tri-borough service in Hammersmith and Fulham, Kensington and Chelsea and Westminster).

You will be able to provide decisive and effective therapy input and will demonstrate leadership qualities, excellent communication skills and have a flexible approach when working under pressure. You will be passionate about the value of community rehabilitation and preventing hospital admissions.

Main duties of the job

This post will give opportunity to experience a wealth of learning and development. You will work alongside and show leadership in a responsive dynamic MDT who put high quality, patient centred care at the heart of what they do. We strive to provide first class, client-centred, goal focused therapy to patients in the community.

The caseload will be split between community and Pathway 3, step down patients within nursing homes with support of MDT. Caseload is comprised predominantly of adults who may present with multiple complex health needs in community. As such, you will have the opportunity to develop a wide range of clinical skills and interests in areas such as discharge planning, falls and frailty, manual handling, orthopaedics, general medical and surgical conditions. An understanding of the management of multi-pathology patients and the impact that psychological/cognitive factors may have is desirable.

You will be based in Hathaway house but are expected to spend time in pathway 3 nursing homes which may be based in tri-borough.

Job responsibilities

You will complete complex assessments and interventions and will make high impact clinical decisions. You will be expected to work autonomously whilst drawing assistance from other colleagues in primary and secondary care.You will use evidence based principles/current best practise and specialist skills to assess, plan, implement and evaluate interventions and to ensure that all members of your team do so accordingly.

As a lead Physiotherapist/Occupational Therapist within the team, you will provide excellent supervision and guidance, to junior therapist and therapy students to deliver evidence based rehabilitation. You will manage your own caseload, providing individual Physiotherapy/Occupational Therapy assessments and access visits to patients in their own homes and care homes. You will lead on manual handling, pressure care and equipment needs within the team and support the team lead. You will deputise for the Therapy lead as appropriate.
