An exciting opportunity has arisen for a motivated band 6 occupational therapist with an interest in supporting discharge planning and rehabilitation to patients that have transferred from the acute sector to the short stay beds based in care homes within Oxfordshire. This opportunity is open to candidates who are ready to progress their career to a band 6 role with the support of our therapy lead or an existing band 6 that wishes to gain valuable experience working with both the acute and community sectors.
Part time working will also be considered.
Main duties of the job
You will be supporting the multidisciplinary team including our patient and families toward discharge. This will include timely assessments and providing rehabilitation plans and providing care planning with the care home staff and our therapy assistants. We complete home assessments with our patients and access visits as required. you will be feeding back to the MDT your findings and outcomes of assessments.

To work autonomously as a member of the multi-professional team supporting patients in the Short Stay Hub Care Home beds.
To provide an Occupational Therapy (OT) service to Short Stay Hub bed patients.
To support Therapy Assistants to manage the rehabilitation and reablement of patients.
To plan and prioritise own caseload.
leadership, guidance, supervision and development for junior staff, unregistered staff and students.
To provide timely specific Occupational Therapy reports and other patient related correspondence according to the quality standards of the service and Professional Standards.
To develop clinically related administrative duties .
To be able to function in a multidisciplinary team identifying unique skills of Occupational Therapists.
To work closely with associated specialities within the hospital and community.
