The Memory Assessment Service operates between the hours of 9.00am and 5.00pm Monday to Friday and the post holder will undertake responsibility of theservice in the absence of the Manager.
The role includes dividing time equally between a clinic and community care setting due to the large rural geographical area.
It is necessary to work autonomously and be in a position to conduct patient assessments in a variety of community settings and have the ability to make complex decisions unsupervised. Expected to actively participate as a full member of the MDT, working with people who present with possible dementia, taking responsibility for a case-load of patients with complex needs. Completing comprehensive initial nursing assessments together with robust risk assessments, delivering and sharing diagnosis and formulating treatment plans, working with families, carers and with wider partner agencies and commissioned services.
The service works within the framework of the All Wales Dementia Pathway of Standards whilst maintaining the National MSNAP Accreditation Standards to ensure high quality diagnosis, care delivery and interventions in dementia within a Primary Care Mental Health Service.Main duties of the job
An exiting opportunity has arisen for a Mental Health Nurse to join us as a Band 6 Memory Clinic Nurse in Gwynedd and Mn Memory Assessment Service (MAS). This is a long established multi-disciplinary MAS providing a service of excellence as part of an Accredited MSNAP Service.
We will provide you with the opportunity to develop transferrable skills - to become specialised in the assessment process by further developing your assessment techniques whilst carrying out in-depth first line nursing assessments collecting and collating information in an appropriate format to present to your MDT colleagues sharing and discussing sensitive information as well as participating in the diagnostic process providing knowledge and support during the early days of diagnosis as well as being involved with initial therapeutic group/ individual work. We want to support you in your career and give you opportunities to progress.
The role will require a mix of nursing skills. As a Memory Clinic Nurse you will be expected to lead an area of the MAS in the absence of the Memory Clinic Manager. This is a unique opportunity to join our team during these interesting times of change and quality improvement initiatives within dementia care within OPMH Services and the implementation of the All Wales Dementia Pathway of Standards.
