The OCE provides specialised rehabilitation for individuals from 16 years of age following a neurological incident the majority of our patients have suffered vascular or traumatic brain injuries as a result of head injury, stroke, subarachnoid haemorrhage or other forms of brain injury from infection or cardiorespiratory arrest.

This is a developing service, in a process of change, and we are looking for staff with enthusiasm who wishes to provide high quality care to individuals in this setting.

The OCE Inpatient Ward admits patients for 3-6 months. We are a Level 1 Neuro-rehabilitation centre. During their admission, the multidisciplinary team (MDT) uses a patient centred approach to patient care and supports patients and their families to give them the best opportunity to grow as individuals, regain independence and reintegrate back into society.

Main duties of the job

We are currently seeking an Admission & Discharge Co-Ordinator to join our Neurological Rehabilitation Service at Oxford Centre for Enablement to contribute to our successful services based in Oxford. Oxford Centre for Enablement has ambitious plans to develop our services, and this is an exciting time to join us .

The Oxford Centre for Enablement is part of the Medicine, Rehabilitation and Cardiology Division this is part of the Nuffield Orthopaedic Centre Oxford University Hospitals NHS Trust.

The Neurological Rehabilitation Centre on the Nuffield Orthopaedic Centre site is an 18 bed level 1 Rehabilitation unit. The Centre provides specialist short-term and long-term person-centred rehabilitation, therapy and support for people of 16 years old and above who have had an acquired brain injury which could be a result of a traumatic incident or illness. Many have complex neurological conditions which cause functional deterioration, psychological needs and behavioural challenges.

Our service offers intense rehabilitation for patients in the post-acute phase following a brain injury right through to step-down independent living. Our patients stay with us from 6 -12 weeks and the rewarding part of the job is establishing a relationship with them and watching them progress through their Rehabilitation plan towards a level of independence.

Job responsibilities

As a Admission & Discharge Co-Ordinator you will be responsible for the admission and discharge of patients on the ward. The candidate should have an interest in Rehabilitation.

Your role will be to provide the team with first class administrative and organisational support that allows us to deliver an excellent admission and Discharge process to our patients and their families.

We are looking for someone confident, friendly and exceptionally well organised to manage this role. In return we can offer you a varied and exciting role in a supportive environment.

Person Specification

Evidence of experience and knowledge

Essential

  • Knowledgeable about current health care issues.
  • Years of extensive experience in complex discharge.
  • Good knowledge and use of English language
  • Good knowledge of Microsoft Office, excel, outlook, power point, EPR and experience of storing information electronically.


Desirable

  • oRegistered clinician,Nurse, Physiotherapist, Occupational therapist candidate with the extensive experience in complex discharge.
  • oExperience of working with individuals with Complex Neurological problems
  • oPractical knowledge of the admission & discharge process within the NHS
  • oAdvanced working knowledge of IT
  • oPrevious administrative , secretarial work
  • oExperience gained within NHS or public sector organisation
