An opportunity has for a band 6 Heart Failure Support Nurse, to be seconded to the HF Team, for 6 months.

The post holder will undertake reviews of Heart Failure Patients within the hospital, alongside the current HF team. There may also be an opportunity to deliver Ferritin infusions to HF patients within the Department of Medicine.

Applicants must have a passion for managing heart failure and be keen to undertake further learning within the field of Heart Failure.

Main duties of the job

Reviewing HF patients within the acute trust. Advising and educating patients, carers to manage symptoms. Be a point of contact for other health professionals as required. Delivering IV iron infusions to HF patients within the department of medicine.

The Heart Failure Nurse team is a proactive team that aims to improve the care and management of Heart Failure patients whilst inpatients and also within the outpatient setting. There are close links to the community Heart Failure Team to improve continuity of care.

Qualifications

Essential

• Diploma / Degree
• Evidence of CPD esp in HF

Desirable

• Recent / Current experience managing heart failure patients

Skills / Knowledge

Essential

• Evidence/ experience of cannulation and delivering IV drugs including IV iron
• Knowledge of NICE guidance for HF

Desirable

• Experience / ability to manage a case load of patients
