We are recruiting for Specialist Therapist for Acute Frailtywho wishes to join #TeamIOWNHS. You should share our passion and drive to ensure that our patients receive quality and compassionate care and work with us to ensure the Trust continues its journey towards great people, great place.

We are delighted have investment in the development of an Acute Frailty service at St Mary's Hospital. The service will be led by a Consultant Nurse/ Practitioner. This Specialist Therapy post is open to both Physiotherapists and Occupational Therapists with a proven history of frailty expertise, clinical and service leadership.

This is a new post created as part of the new acute frailty team working primarily within the emergency care floor, with in-reach into acute wards, assessing needs, providing same day services, and supporting the effective use of alternatives to acute care including SDEC, virtual wards and community rapid response services.

The creation of an acute frailty team contributes to the Trust's strategic objective of Home First - enabling people to remain safely in their normal place of residence - by better meeting the needs of people aged 65 and over living with moderate to severe frailty.

Job description

Job responsibilities

  • To become a member of a high functioning team Acute frailty delivering a quality comprehensive service to patients with Frailty.
  • To work under direction of the specialist therapist to provide frailty assessment, clinical care and treatment of patients within the acute frailty pathway.
  • Under direction of the specialist therapist this post will work primarily within the emergency care floor and in-reach into acute wards, liaising appropriately with acute therapy teams in order to assess needs, providing same day services, as well as supporting the effective use of alternatives to acute care including SDEC, virtual wards and community rapid response services.
  • Under direction of the specialist therapist for acute frailty, contribute to the delivery of frailty awareness activities across acute service.


Person Specification

Qualifications and Experience

Essential

  • Physiotherapy/Occupational Therapy Degree/Diploma
  • HCPC Registration
  • Evidence of CPD at a postgraduate levels as well as, reflective practice, IST and experiential learning
  • Experience of working as part of a therapy team/multidisciplinary team. Team management experience
  • Extensive general and acute experience and experience of working as a clinical specialist.
  • Experience of student and staff clinical education
  • Understanding of legal responsibilities of the profession.
  • NHS or equivalent clinical experience
  • Evidence of participation in research/audit


Desirable

  • Working towards MSc or equivalent postgraduate qualifications.
