The urgent community response (UCR) team is a 7 day service that responds within 2 hrs to the needs of patients in their own homes, including nursing and residential care homes, to avoid admission to hospital. The team are also providing support to patients that are on the virtual ward.
The service brings an initial holistic assessment to the patients home within two hours, identifying health or social care needs and developing assessment and treatment plans to come up with appropriate differential diagnoses which may include doing a range of diagnostic tests.
We are looking for experienced paramedics or nurses with a passion for patient care and the drive, enthusiasm and talent to help lead a team. You will want to develop your skills and push yourself outside the normal boundaries of practice and will have a real drive to make sure your patient gets the best treatment possible.
Main duties of the job
The post holder will be an experienced Registered Nurse in Adult Nursing or a HCPC registered Paramedic who will work across the urgent community response services. We would encourage applications from professionals with high level clinical skills, knowledge and experience in a constantly evolving environment.
As a Band 6 urgent community response practitioner you will: Provide good leadership to the team on shift and support the development of the service through leading on projects and audits. Be the clinical coordinator of the team on shift, offering excellent customer service. Attend patients to do a rapid initial assessment and for follow up visits. Develop your clinical skills into a more well-rounded holistic approach to patient care. Develop new skills outside of your normal scope of practice including skills from Paramedicine, Nursing, Physiotherapy and Occupational therapy.
The urgent community response team works 7 days a week covering shifts between 8:00 until 22:00. There is also potential of some night shift cover as the service expands. You will be expected to work weekends and bank holidays.
