Do you want to join an outstanding team of community nurses and therapists in Newham? We are looking for a B7 District Nurse to join our team who is passionate about community nursing and making a difference to people's lives. You will be a strong team player who is skilled at inspiring a team of nurses to perform at their best in support of patients and the team in line with ELFT's values. You will be committed to working as a keyworker for your designated patients, ensuring that you review them at least once a month in line with our keyworker project.
Main duties of the job
You will play a vital role in ensuring the service operates at the highest level, e.g. by attending the monthly management , pressure ulcer and safeguarding meetings. You will also be able to put into practice and develop your clinical and managerial supervision skills and engage in QI projects.
About us
You will be supported by a wide number of healthcare professionals, including DN and therapy clinical leads, a deputy lead nurse and lead nurse, pressure ulcer improvement facilitators, palliative nurses, a referral and assessment team, a rapid response team, care navigators and telehealth practitioners. You will receive monthly supervision with your clinical lead and participate fully in a monthly senior nurses meeting.

To have responsibility for the nursing care given to the patients on her/ his own case load and for the nursing care provided by the team. The District Nurse Team Leader will ensure that the case management model of delivery, including the effective assessment, planning, implementation and evaluation of care and the discharge of patients from the caseload meets the professional and clinical standards/guidelines at all times. She/he will have line management responsibility for the human resources of the nursing team, the services provided by the team and will be professionally accountable for the appropriate delegation of patient care to nurses within the team and for the supervision of their work and will monitor expenditure within the team. The District Nurse Team Leader will ensure that a high quality service is provided in patients homes, clinics or residential care settings within available resources
