We are looking for a Band 5 Staff nurse with a passion for care of the older person to join our established team. Ward 12 is a 23 bedded Sub-Acute Elderly care ward. We care for patients aged 65yrs and over with a number of different complex medical needs. We have a large Multidisciplinary team who we work closely with on a daily basis co-ordinating complex and often challenging discharges.
Patient safety and the care of patients with cognitive impairments is an essential focus on the unit and we strive to provide excellent patient care in these areas.Main duties of the job
Planning , evaluating and implementing the care of a team of 12 patients on a daily basis. Co-ordinating complex discharges safely. Supporting junior staff including TNAs and Student nurses. Keeping up to date with relevant training.About us

To assess, plan, implement and evaluate nursing care working as a member of the ward team under the direction of the Ward Manager.
To be responsible for the organising and co-ordination of nursing services in the ward/dept in the absence of senior staff on a regular basis.
