Hywel Dda University Health Board has a new, exciting opportunity for a motivated and driven Physiotherapist to join our Community Rehabilitation Team in Llanelli, and surrounding areas of Carmarthenshire. The Physiotherapy Community Team is rapidly growing and evolving. This additional investment has highlighted the Health Board's commitment to improving access to rehabilitation as close to home as possible for our population.

The post holder will work closely as part of a broader, larger, dynamic multi disciplinarily teams (MDT) of occupational therapists (OT's), nurses, social workers, third sector services, Community Hospitals, Front of House services (TOCALS), Acute Crisis Response service, Reablement, Home Based and Bed based rehabilitation services, and our acute colleagues to provide timely, patient-centred care seamlessly across different settings. The role covers a variety of patients and pathologies including those with neurological conditions, cardiorespiratory conditions, musculoskeletal (MSK) problems, falls and frailty.

Main duties of the job

Most visits are face to face at home, however we are also embracing digital assessments. The post holder will also participate in and help develop community-based classes, and Rehabilitation Outpatients. The post holder will help to lead service development. The post holder will be required to participate in the provision of aflexible 7-day service, based on service need and rota basis. They will have the skills required to participate in the respiratory On-call service and Weekend in-reach.

The applicant will need an excellent ability to manage and prioritise a diverse caseload, support and supervise Physiotherapy staff and have the ability to travel between sites and in the community in a timely manner

  • Professional registration as recognised by HPCP

Experience

  • Post-graduate experience in relevant clinical specialty
  • Completion of junior competency framework
  • Practical experience to a level that would enable participation on the respiratory on call rota
