Nurse Team Lead for Intermediate Care at Home and Discharge to Assess

This is an opportunity for staff with experience managing discharge to assess services to join our Stockport team.

To hold day to day responsibility for managing the provision of clinical services within a defined work area.

Follows broad occupational policies and implements within sphere of work.

Deploy and manage human and financial resources to ensure safe and responsive clinical services.

Main duties of the job

Manage the deployment of the ICaH and D2A teams, considering skill mix recruitment and flexible working patterns.

Provide support for the team across working hours to manage expected and unexpected clinical events within the scope of practice.

Job description

Job responsibilities

Supervise and monitor staff rostering and skill mix to maximize effective delivery of care.

Ensures risk assessments are completed and actions implemented to meet the health and welfare needs of staff and patients.

Ensure national and local targets are reported through the organisations reporting mechanisms.

To regularly monitor and report on quality assurance within organisational frameworks.

Act as a professional resource to other healthcare professionals providing support within their scope of practise.

Assess, plan and implement packages of clinical care to patients/clients within own sphere of practice in a variety of community settings.

If successful this vacancy will require an enhanced DBS check at a cost of �41.90. This cost will be deducted from your first Trust salaryIn addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of �13.

Person Specification

Education and Qualifications


  • 1st or 2nd Level Registered Nurse
  • V100/V150 Prescriber


  • Community Specialist Practitioner or equivalent.

Work Related Circumstances


  • Occupational Health Clearance
  • Car Driver
  • CRB Clearance



  • Has a broad range of specialist knowledge within sphere of responsibility.
  • Able to demonstrate leadership development.
  • Knowledge of Government legislation and guidance pertaining to the role.


  • Knowledge of Trust policies and procedures.



  • Experiences in the management of a D2A/ICaH
  • Experience in the management of human and financial resources including unexpected fluctuations in workload.
  • Experience of managing change.
