A wonderful opportunity has arisen for a highly motivated, experienced nurse to join our Cardiac Rehabilitation team to be responsible for ensuring that patients referred for cardiac procedures/ surgery are contacted in a timely manner and options of care are discussed fully and then ensure the patient's pathway is followed and all tests/ investigations are organized appropriately.

Providing expert nursing advice, care and support to patients, their families and carers working as an integral member of the Cardiac Rehabilitation team.

Your role is integral to the smooth transition for patients who have received a cardiac diagnosis and are required to have further cardiac intervention. you will be the first port of call to discuss this diagnosis with the patients and/ or family and organise all investigations/ referrals etc to ensure that the patients do not breach the 18 week pathway.

Therefore you must have relevant cardiac knowledge and have worked in a cardiac setting as well as having excellent communication skills as communication with patients, families, staff and stakeholders is essential.

Main duties of the job

Your role is to assist in the co-ordination of an effective high quality Cardiac Rehabilitation service including the education, emotional and psychological support of patients, relatives and carers enabling them to live with their disease, treatment and the implications thereafter.

You will support the Clinical Nurse Specialist in monitoring and improving standards of care through supervision of practice, clinical audit, developing and leading practice, contributing to research, teaching and supporting professional colleague and liaise effectively to establish and maintain professional relationships with the multi-disciplinary team, agencies and organisations. To represent the Trust and work in partnership with colleagues locally and nationally.

You will assist with implementing the cardiac choice initiative enabling patients to make informed decisions about future procedures, organise appropriate tests and investigations for patients referred for cardiac procedures to ensure a seamless patient pathway and assist in maintaining a robust referral system for cardiac surgery and cardiology referrals.
