We are looking for a Senior Operational Clinician with excellent communication and people skills who can provide the best leadership and management support to our Urgent Community Response (UCR), and frailty virtual Ward (VW) team in the Basingstoke area.
Due to partial retirement, we are recruiting a motivated and inspirational individual to work alongside the current post holder to create a culture within your team where transformation, change and service development are the norm.
We want you to lead and support the team in the development of effective relationships with both internal and external colleagues, keeping in touch with what is important to others, and must demonstrate strong clinical skills and ideally experience working in and/or understanding Community services.
The team provide a model of care to provide urgent care and Frailty virtual wards and focuses on preventing avoidable hospital admissions for patients with acute ill health conditions which includes implementation of treatment and appropriate monitoring whilst in crisis. Our primary aim is to provide first class health care, in the home, giving the service users the choice to remain at home and recover more quickly.
This is a part-time post (3 days initially) and will be based in Herriard, preferred days will be Mon- Wednesday 8-4pm, and can be discussed at interview.
Main duties of the job
The primary role of the Senior Operational Clinician for Urgent Community Response and Frailty virtual wards is to provide strong holistic leadership to the team. To include day to day leadership responsibilities, development, and creative, responsive leadership, enabling the team to achieve a 'well led' culture.
The Senior Operational Clinician, in conjunction with the Consultant Practitioner, Senior Clinical Practitioner and Advanced Clinical Practitioners, GP's, Practice Nurses, and Social Care professionals, will support and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.
The Senior Operational clinician will participate in and influence efforts across health trusts and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management, and assure early intervention through the proactive provision of care in or as close to the patient's own home as possible.
