We are looking for an enthusiastic RMN/Social Worker/OT to join our friendly Multi-disciplinary team as a Care Co-ordinator. You will require experience of working in a mental health setting and awareness of delivery of community mental health care.

Main duties of the job

As a care co-ordinator within Central East Community Mental Health Team, you will provide person centred, recovery focused and inclusive support to the community. We work with individuals with complex mental health and social care needs in a variety of community venues in addition to seeing people in their own homes.

The service calls upon a wide range of evidence based holistic assessments and interventions to support the individual at the heart of our care. As part of your role you will also work jointly with carer's, family members and any other individuals identified by the service user as key to their recovery.

As a Community Mental Health Practitioner you will be required to undertake the role of Care Coordinator in line with CPA process. You will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway and will be expected to work proactively and flexibly within a multi-disciplinary team model.

There is a requirement and opportunity to develop your clinical leadership skills and to assist in service development, Service User engagement and improving the wellbeing of our communities.

You will also be expected to act as assessor/mentor for students.
