This is an exciting time to join the CYP Crisis Service in Wigan and Bolton. The service is entering an evolving and developing phase and would be a great opportunity for a dynamic and enthusiastic individual to effect real change in the way in which CYP, families and carers receive mental health crisis support.
The successful candidate will play a central role in supporting the clinical and operational team to ensure high quality, recovery focussed care is delivered to our service users.
The post holder will hold a complex caseload and will lead in risk assessment and management, formulation and prescribing of care / treatment planning, including discharge planning within agreed protocols and procedures and be an integral part of the wider CAMHS MDT to support evidence based clinical decision making. The successful applicant will support service pathways in developing nurse led teams. The post holder will also support the development of the service by driving quality assured improvement to provide a meaningful, seamless and recovery focussed patient journey. They will do this with evidence based therapeutic interventions, to promote resilience, recovery and self-management, as clinically appropriate.
Main duties of the job
The post holder will provide advanced and expert level of practice (including non-medical prescribing) within the CYP Crisis Service leading to improved patient outcomes and reducing the need for Tier 4 admission or presentation at A& E without medical necessity.
They will be an integral part of the MDT supporting clinical decision making and supporting the service pathways, developing nurse led clinics and providing brief interventions.
The Advanced Practitioner will hold a complex case load and will lead in holistic, collaborative risk assessment and management, formulation and prescribing of care planning including discharge planning within agreed protocols and procedures. They will also provide guidance and assistance to the MHPs in the team in order to provide a safe governance structure.
The job holder may additionally lead on practice and service developments. The job holder is required to demonstrate clinical and practice expertise via interventions, supervision, modelling good practice and challenging change.
