The Trauma Rehabilitation Coordinator will assist with rehabilitation assessments of patients admitted with trauma and will, with the other professionals involved develop a rehabilitation prescription/plan. He/she will ensure that goals are communicated to patients (where appropriate), to their families and to all the acute and rehabilitation team members. He/she will review progress and along with the wider trauma team determine the best timing for transfer to the most appropriate rehabilitation facility for further treatment. He/she will have knowledge of other rehabilitation services, such as neuropsychiatry and clinical psychology, outpatient services, specialist nursing homes and will liaise as appropriate with these facilities and professionals. The post holder will help to coordinate team assessments across a range of disciplines. The post holder will also have access to support and supervision at all times from a senior Rehabilitation Coordinator and Consultant in Rehabilitation Medicine.

Main duties of the job

The Trauma Rehabilitation Coordinator will, in conjunction with other senior members of the trauma team, provide specialist information, advice and support to patients, relatives and other professionals, which at times may be of a sensitive and complex nature. The post holder will update his/her knowledge of the health status and rehabilitation prescriptions for trauma patients and of appropriate rehabilitation facilities. The post holder will assist in the planning, development and coordination of comprehensive rehabilitation programmes in conjunction with the patient, relatives, key members of the rehabilitation team, ward staff and also other services/agencies where necessary.

Education & Qualifications

Essential

  • Basic professional qualification, e.g. Registered Nurse, OT or Physiotherapist
  • HPC or NMC Registration
  • Additional post registration study
  • Evidence of personal development and commitment to continual professional development

  • Mentorship qualification
  • Counselling skills


Experience

Essential

  • Minimum of 3 years post-graduate experience
  • Relevant specialist post-graduate training/study and in-depth experience in a range of specialist areas
