This exciting new clinical leadership post has been developed to bring together our newly established Virtual Hospital (VH) and Urgent Community Response Services (UCR) for West Essex. These services are an integral part of the West Essex out of hospital care model designed to ensure that patients receive rapid appropriate care and treatment as close to home as possible. The post holder will have overall responsibility for the day to day clinical operational management of responsive services which are aligned to the Care Coordination Centre. They will work autonomously in assessing, diagnosing, treating and evaluating care for adults requiring complex management with the aim of preventing inappropriate hospital admission and facilitating prompt discharge from acute hospitals. Integral to this role is working closely and establishing effective professional relationships with patients, relatives and partner organisations.
The post holder will require specialist knowledge to demonstrate safe, competent clinical decision making and expert patient care including diagnostic skills. They will communicate and work collaboratively with the wider MDT to meet the needs of patients, supporting the delivery of policy and procedures and ensuring a cohesive service that is aligned to high quality care providing strong leadership within the team and local area. This will include adopting system-wide approaches required to perform as best practice.

Main duties of the job

KEY RESPONSIBILITIES
Leadership
- Provide a visible, accessible and professional leadership presence for the team, service users and their families.
- Be responsible for the safe and effective delivery of VH & UCR services within West Essex community.
- Ensure and embed high standards of care and safe practices in line with national guidance and best practice. This includes ensuring best practice arising from national reviews is fully embedded in local practice.
- Represent the team and Organisation at local system and Integrated Care System (ICS) meetings/events relevant to the delivery of virtual hospitals and urgent community response
- Build a strong alliance with our system-wide partners to ensure representation and service development can be maximised.
High Quality Clinical Service Delivery
- To have a duty and responsibility to act within their professional boundaries. In order to work at this level NMC / HCPC requirements for advanced practice must be met
- The post holder will provide clinical expertise and leadership to the VH & UCR integrated clinical team, drawing on clinical knowledge to undertake rapid interventions to include advanced assessment, diagnosis and treatment, inclusive of independent prescribing
- Rapidly respond to and triage new referrals, assessing clinical condition, care environment and treatment options
- Working integrative with the wider health and social care team within the Care Coordination Centre to meet the needs of patients, ensuring patients receive the right care, by the right person at the right time and in the right place. Placing the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, with minimal avoidable delays, whilst improving the quality of care preventing unnecessary hospital admission whilst also supporting safer discharge from hospital.
- The post holder will demonstrate critical thinking in the clinical decision-making process.
- Be accountable for planning and managing total episodes of care, working closely with the wider Care Coordination Centre health and social care team. Communicating effectively with patients and their carers to maximise recovery potential and to ensure understanding of condition by providing both written and verbal advice.
- Ensure co-ordinated care is patient focused and underpinned by evidence based practice and continual professional development to enhance the knowledge and skills of staff.
- Provide and receive complex, sensitive or contentious information requiring the use of developed negotiation, empathy, persuasive and motivational skills
- Support systems for measuring and improving the quality of the patient/care experience
- In collaboration with the Integrated Service Manager, ensure there are systems in place to learn from all patient/carer experience safety.

Team Development
- Oversee the production of team development plans that ensure that all staff aligned to VH & UCR services have (a) achieved the required competencies for their role and (b) have opportunity to access additional training and development in line with their appraisal personal development plans.
- Support the implementation of extended clinical practice to modernise the service, to benefit patients and their families. Support the progression of the services and ensure relevant redesign is supported to benefit patients and their families care.
- In collaboration with senior clinical colleagues and the system, develop and review workforce planning including annual reviews of skill mix to ensure a high performing, safe and effective service.
- Ensure the VH & UCR teams meet their responsibilities to provide clinical experience and mentorship support for pre and post registration students (future possibility of non-nursing student placements, social workers, therapists), in line with internal and external standards.
- Ensure a system is in place to analyse the training and development needs for VH & UCR staff.
- Support the service to become a high performing and clinically recognised centre of excellence.
Governance
- Undertake investigation and management of incidents in relation to the service.
- Manage complaints relating to the service in line with agreed Trust processes and where required to resolve complex complaints, meeting with complainants to address their concerns to ensure local resolution is achieved wherever possible.
- Ensure implementation of effective and high quality systems throughout the service, liaising with other agencies around Safeguarding, human rights and best interest decisions.
- In collaboration with the senior management team, support the Directorates approach to improving compliance with NHSLA standards, CQC registration and other regulatory requirements.
- Work with the senior management team to ensure the delivery of the Directorate and Corporate Objectives.

Essential
- Registered Nurse with current NMC registration
- Post Registration Qualification in Advanced Practice
- Degree level education in relevant subject or equivalent intellectual ability
