Work with the Healthy Child Programme (HCP) 0-5 Team to provide a child centered, inclusive, holistic and accessible public health programme to children, their families and communities.

Establish effective links with children, families, communities, statutory and voluntary agencies.

Promote the physical and emotional wellbeing of children and young people, in order to establish good health for them now and in the future through early intervention, health promotion, advocacy, empowerment, social inclusion and the reduction of health inequalities. Make referrals as appropriate enabling access to relevant professionals supporting children and young people to reach their health and developmental potential

Main duties of the job

Contribute to the work of the team under the direction of the team lead.

Actively promote the health of children and young people by participating in the Healthy Child Programme as set out in the service specification.

Facilitate students of nursing to develop their own competence as per NMC code of professional conduct.

Submit accurate data and statistical returns within defined timescales and contribute to the maintenance of accurate computerised databases as requested.

Initiate and contribute to Early Help Plans as per Healthy Child Programme under supervision of a named SCPHN.

Within competence, review children's health, including growth, immunisation history, nutrition and dental health, identifying health needs and offering appropriate follow-up.

Provide evidence based health promotion advice on a range of subjects.

Deliver identified training sessions.

Respond to child protection and safeguarding concerns & report appropriately.

To attend, contribute to, document and report at child protection case conferences and planning meetings, making an assessment of risk with the supervision of the school nurse.

To ensure health monitoring equipment is operating to a high standard, maintained and calibrated.

Job responsibilities

Communicate effectively with the HCP team, other professionals, parents and children, within the bounds of confidentiality.

To adhere to the Pan Sussex Child Protection Procedures and Sussex Community NHS Foundation Trust (SCFT) safeguarding procedures, attending meetings and partnership working as required.

To attend regular one to one, clinical and child protection supervision.

Deliver an effective service, by developing good written, verbal and IT skills.

Ensure high standards of communication with other professional organizations.

Participate in the SCFT annual Performance and Development Review process and engage in the development of a professional development plan.

Following completion of portfolio of skills competencies, take responsibility for own development assessing and identifying professional development needs using feedback from supervisor, team lead and the HCP team.
