We currently have an exciting opportunity to join the Children's Community Nursing Service in our Inner London Division We are seeking a highly motivated and enthusiastic Band 7 Children's Community Nurse Team Lead who islooking to share and develop their skills in the largest community healthcare trust in London
The post holder will work as part of the Children's Community Nursing Team, taking overall responsibility for the assessment, management and evaluation of evidenced based nursing care to children and their families or carers. The post holder will also work in conjunction with 2 other Team Leads and the Specialist Nursing Service Manager in managing the resources of the Team, including staff, in line with professional standards and quality of care and provide managerial cover in the absence of the Specialist Nursing Service Manager. You will work closely with the Children's Community Nursing Team, Social Services, and hospital teams.
The team provides a service to children and young people aged 0 - 18 years, and works across the 3 London Boroughs of Kensington and Chelsea, Hammersmith and Fulham, and Westminster. The team works closely with colleagues in Primary Care, hospitals, social services and education.
Main duties of the job
This post is for 18.75 hours per week, which will be worked flexibly over 7 days a week. This post offers a great opportunity for an experienced children's community nurse to develop and enhance leadership skills and experience in community nursing. The organisation is supportive of training needs and there is an excellent mandatory training and education programme. You will also receive regular clinical supervision and safeguarding supervision.

To take continued responsibility for the assessment, management and evaluation of skilled nursing care in negotiation with the child and family To work in a collaborative manner with other agencies to ensure a seamless delivery of care to families To provide information and support to facilitate the childs and familys own choices with regards to nursing care To promote Health Education as an integrated aspect of care delivery To lead complex discharge planning and assessment of a childs needs, ensuring detailed communication with discharging hospital and safe discharge for the child and family To maintain accurate, systematic and timely record keeping including recording of clinical data.
