Come and join a friendly and enthusiastic community adult speech & language therapy team based in the London Borough of Hillingdon.You'll provide assessment & therapy to a range of acquired & progressive neurological disorders consisting of communication, swallowing, voice + stammering.

This is a flexible split post in the community & outpatient setting. You'll be part of an MDT with physios, OTs and dietitians. You'll work as an autonomous practitioner when completing domiciliary & nursing home visits. There's involvement in delivering dysphagia training programmes to nursing homes & agencies.

We're happy to consider supporting a Band 5 therapist including an NQT,to develop into the full Band 6 role.You'd have an initial fixed term contract at Band 5 with a robust development framework in place to take you up to band 6. You'll work with band 7 lead therapists in each clinical specialty to support & develop your skills. There's access to regular supervision & opportunities for internal & external CPD. Regular team meetings, support & supervision are in place, plus good opportunities for training & career development.

Main duties of the job

• The independent management of a caseload of clients with swallowing and communication disorders in the community and in an out-patient setting.
• Close working relationships with all members of the multi-disciplinary team
• The active contribution to service development in SLT service within the areas of stroke, long term neurological conditions & older people services
• The delivery of training/teaching to carers, other health professionals and students

Job responsibilities

The post holder will provide a Specialist Speech & Language Therapy service as an autonomous SLT working with the Community Adult Rehabilitation Team (which includes other SLTs), for clients who require domiciliary visits or can attend out-patient appointments.

The postholder will be part of a community SLT service providing specialist assessment and treatment to adults with communication and swallowing difficulties within the areas of stroke, progressive neurological conditions, and older people within clients own homes, residential homes and nursing homes.

The postholder will provide comprehensive assessment and treatment of In-patients in an Intermediate Care Unit and outpatients and ensure a seamless pathway from inpatient to outpatient status and where appropriate in long-term conditions follow-up in the Community setting.

The post holder will demonstrate efficacy of this service to stakeholders with appropriate liaison and relevant audit. The post holder will access support from other Community based Band 6 SLTs, and supervision from the band 7 SLT and the Adult SLT team lead.

The post will also rotate into voice out-patient service with specialist supervision in these areas in order to develop the relevant clinical competencies.

• Provide culturally competent practice in all interactions with patients and in the preparation and design of all clinical interventions

Qualifications

Essential

• Recognised Speech and Language Therapy Degree Qualification or equivalent
• Health Professions Council - Licence to Practice

Desirable

• Registered Member of Royal College of Speech and Language Therapist
• Evidence of successful completion of specialist short courses up to Master's degree equivalent
• Membership of relevant Clinical Excellence Networks

Experience

Essential

• Substantial experience working with adults with acquired and progressive neurological disorders in a variety of settings.
• Relevant highly specialist experience post qualification in assessment and management of acquired communication/swallowing disorders of neurological aetiology
• Involvement in multi-disciplinary working

Desirable

• A significant proportion of post graduate experience should include community setting
• Independent in videofluoroscopy
• In-depth experience within an appropriate specialist area (adult acquired and progressive neurological disorders, dementia).
• Experience of supervising and appraising more junior therapists
• Experience working with interpreters
• Experience of developing and providing specialist training to nursing home staff, carers, and other members of the MDT
